Requirements for Receiving Your Original Certificate:

• Produce the receipt of the return of convocation materials, i.e., gown, cap, and hood. (Please collect the receipt from your respective department.)
• Return the provisional certificate if issued earlier.
• Graduates with outstanding dues (accounts or library) must fill out the application form available at the respective department or download the form from the following link: https://www.uap-bd.edu/services/f5.pdf
• Graduates who have applied for provisional certificates and transcripts but did not receive them must return their student ID cards.
